Free and Discounted Sports Physicals

7th and 8th graders are required to pass a sports physical prior to participating in school sports. Sports physicals are valid for two years. There are four opportunities for free or discounted sports physicals over the summer:

1. Saturday, July 18th; 8am - 12pm: Free sports physicals for middle and high school students by Salem Health Orthopedics
   • Address: 1600 State St, Salem OR 97301
   • Please register via their Sports Physical Event Page, and bring a parent or guardian (if student is under 15) and a completed OSAA form (can be accessed on the OSAA Website).
2. Thursday, July 30th; 2 - 6pm: $10 sports physicals will be offered along with other medical care (including dental screenings, immunizations, blood pressure checks, and more) for the whole family by Yakima Valley Farm Workers Clinic
   • Address: Lancaster Family Health, 255 Lancaster Dr NE, Salem, OR 97301
   • Family activities, giveaways, entertainment, and food will also be provided for free.
3. Friday, August 14th; 8:30am - 11:00am: Free sports physicals for students preparing to participate in school athletics, hosted by Parrish Middle School

4. All summer long: Free sports physicals offered by Salem Free Clinics
   • Address: 1300 Broadway St NE, Suite 104, Salem, OR 97301
   • The clinic is open on the following days:
     • Monday - Thursday, 8am to 4pm;
     • Friday, 8am to 12pm;
     • and two Saturdays per month.
